Disposition authorization submission process

This process is to be completed by the funeral home of record.


  1. Download an APPROVED proof copy of the electronic death record from WHALES. (Demographics and Cause of death approved PDF preferred).
  2. If religious or cultural ceremonies are planned to take place within 24 hours of death, please complete the DA request and call our office at 564.397.8405 ext 3. This includes weekends and holidays.
  3. Requests are processed within a 24-hour period on business days (Monday-Friday), except holidays. Requests received after 2:30 pm will be processed the next business day. If you have not received a response from the Medical Examiner’s Office within one business day of submission, please call the office at 564.397.2595. Do not resubmit your request as this could potentially cause you to be billed twice.
  4. The MEO makes every effort to process and approve all DA requests as efficiently as possible. However, requests may be delayed if information provided on the death record is incomplete or do not meet state guidelines. To prevent delay, please ensure all sections of the death record are complete prior to submission to the MEO.
  5. The Funeral Home/Disposition company listed on the death record will be billed $100.00 on the 10th of the month proceeding the approval.
  6. Complete the Request for Disposition form, attach the approved proof copy and then submit.


**NOTE: If the funeral home of record changes following the approval of the disposition authorization, it is the responsibility of both funeral homes to notify the Medical Examiner's Office immediately to avoid errors in billing.

How long it takes for approval

Requests are processed within a 24-hour period on business days (Monday-Friday), except holidays. Requests received after 2:30 pm will be processed the next business day. Please do not re-email your request as this could potentially cause you to be billed twice.

Possible delays when processing requests

The Medical Examiner's Office makes every effort to process and approve all disposition requests as efficiently as possible. However, requests may be delayed if information provided on the death certificates are incomplete or do not meet state guidelines. To prevent a delay, please ensure all sections of the authorization form and death certificate are complete prior to submission to the Medical Examiner’s Office.
